The process starts when a potential client or their family member contacts Upton Care Plus LTD with a service enquiry. The agency provides initial information about the services offered and answers any preliminary questions.
A preliminary phone assessment to understand the potential client's care needs.Learn about the client's health status, daily living activities, mobility, medication, and personal preferences.
Following the preliminary assessment, an in-person visit is scheduled, ideally at the client's home. This allows a comprehensive needs assessment to be carried out. It also provides an opportunity for the client and their family to ask more detailed questions.
Based on the comprehensive assessment, a personalised care plan is developed. This care plan outlines the services to be provided, frequency of care, goals of care, and any specific instructions pertaining to the client's unique needs.
Care services commence as per the agreed care plan. This could be immediate or at a future scheduled date. Regular reviews are carried out to ensure the services continue to meet the clients needs.
